Recently, I got a case of pseudophakic glaucoma developed in the immediate postop period not responding to conventional line of management, ultimately needing explantation and PKP with re implantation.

Please enlighten with the various possible causes, taking into consideration...small incision technique, no pupillary block, open angles, aspirated visco, anteriorly angulated latest design IOLs, minimal surgical insult.
