This 50 yr old labourer developed spontaneous pnemoencephalocele and presented with sudden severe headache which went on increasing rapidly. You can see the break in the post wall of the frontal sinus and the communication. There was no history of trauma at all. Yes, he had lifted a wt. of around 100 kg on his head when this happened. I had no choice but to operate and close the defect in the dura underlying the bony defect and remove all air by saline irrigation. Have any of you any experience with such incident. Please give your feedback.
