one thing you have not mentioned if you had undergone a fistulectomy earlier. anyway, even if not operated earlier, fistula/sinus tract may be single or multiple which may be ramified inside, a very good probing under anaesthesia may help in perfect diagnosis at the same time you may be adviced sinography. sometimes blind tracts be open after primary threading but it usually takes not more than one month. so, if the problem recurred after 6 months' of treatment then you shuod undergo a sinography for further management, there might be a blind tract left behind earlier.
