this child basically had a acute febrile illness with localization to CNS. Differential diagnosis in this setting should include 1. acute bacterial meningitis(favoring points- high grade fever, high counts) 2.cerebral malaria (some details like chills, geographic area, presence of splenomegaly may be helpful) 3.viral encephalitis some details in the history, examination and management would throw light on the exact nature of the disease and the coure of events. were the convulsions focal? any neuroimaging done? what antimalarial was given? what antibiotics were given? at what doses? how was brain death diagnosed - were apnea testing and EEG done, CRAG done?
